Permission to administer First Aid(Required)

I give permission for my child to receive first aid, including over the counter medication, by a designated member of staff according to the School’s Medical protocol.

Permission to act on behalf of parents in the event of an emergency(Required)

I give permission for the course coordinator to give consent to any dental, optical, medical or surgical treatment, including anaesthetic or blood transfusion required in an emergency by my child, if the school is unable to contact parents/guardians.

By submitting this form, you consent to Sevenoaks School collecting and using the information you provide for the purpose of registering you and your students for the Model COP conference.

It will be accessed only by staff responsible for organising the event and will not be shared with third parties.
